FTEHCA Florida Mercy HospitalMiami, FL DESCRIPTIONThe RN CM is
responsible for promoting patient-centered care by coordinating the
plan of care for the patient stay, managing the length of stay,
ensuring appropriate resource management and developing a safe
appropriate discharge plan in collaboration with the
multidisciplinary team. The RN CM facilitates the progression and
transition of care using established criteria and in conjunction
with the multidisciplinary team. The RN CM will coordinate
activities that promote quality outcomes and patient throughput
while supporting a balance of optimal care and appropriate resource
utilization. RESPONSIBILITIESDemonstrates responsibility and
accountability in the performance of assessment, reassessment,
development and implementation of the patient's plan of care in
relationship to the discharge needs and follow-up care and in
accordance with the utilization management plan.Proactively
communicates with the physician and other members of the health
care team to determine the best options to meet the patient's needs
in an efficient and effective manner.Review all assigned cases
within 1 work day of the admission or request for review utilizing
current and accurate clinical case review criteria.Promotes the
most effective and efficient use of resources to achieve the
targeted length of stay.Assesses, collaborates and coordinates the
patient's discharge planning needs and readiness for discharge with
other members of the healthcare team and the
patient/family/SO.Complies with the departmental policies and
procedures related to required processes including but not limited
to documentation, interdisciplinary communication, IM delivery,
teamwork, fiscal responsibility and performance improvement
activities.Assists Physicians in the decision of proper status
placement. Informs and educates internal personnel for correct
status assignment. Works with physician advisor to assure InterQual
criteria are applied.Refers to social worker the high-risk
discharges e.g. homeless, HIV, DCF, Baker Acts, new diagnoses and
difficult transfers/discharges/placements.QUALIFICATIONSAssociates
degree required, BSN preferredHospital Nursing experience
requiredCase Management experience a plusCritical Care experience a
plusCertification in Case Management or Utilization Review
preferredInterQual experience preferred This role requires you to
be fully vaccinated for COVID-19 based on local, state and /or
federal law or regulations (unless a medical or religious exemption
