Duties

The Nursing Assistant Community Living Center, CLC is self-directed in providing nursing care to a variety of patients/residents of ages across the adult continuum The Nursing Assistant to meet personal hygiene needs such as bathing, lifting, turning and feeding; administering tube feeding via feeding pump to include changing administration sets, all the while explain procedures to patient/and or family. The NA provides nursing care, performs clinical procedures according to specialty, technical procedures and ancillary services for complex patient. The NA performs a variety of personal care or nursing care activities. The NA helps each patient attain optimal physical, physiological, emotional and social well-being.

The Nursing Assistant may perform these duties in a variety of health care settings such as outpatient, inpatient, home or community living care, acute, sub-acute. The Nursing Assistant will care for patients/residents with acute to chronic mental health conditions. Performs nursing procedures within scope of practice and according to policies and standard procedures to care for the veteran for such procedures but not limited to: enemas, catheterization, colostomy care, skin care simple dressing, application of braces and prosthetic devices, monitoring of risks on small group activities with veterans/residents according to the patients/resident treatment plan. Duties may also include transport activities (transfer of patients to and from procedures/units) and delivering lab specimens to laboratory.

The Nursing Assistant, works in close collaboration with an assigned RN/LPN to provide safe and quality patient care. The nursing assistant obtains vital signs, assesses individual patient rooms and unit environment for safe/sanitation conditions, stocks as needed and performs equipment checks every shift. The Nursing Assistant is self-directed in providing nursing care to a variety of patients/residents of ages across the adult continuum to meet personal hygiene needs such as bathing, lifting, turning and feeding. The NA helps each patient attain optimal physical, physiological, emotional and social well-being.

The duties and responsibilities of the CLC Nursing Assistant include but are not limited to the following:

• Measure and record vital signs, intake and output, and weights
  
• Meets personal hygiene needs such as bathing, lifting, turning and feeding
  
• Performs EKG testing
  
• Participates in maintaining a clean, safe environment for the patient/resident
  
• Assists with the discharge process as needed to maintain patient flow
  
• provides appropriate hand off report on assigned patients
  
• Makes observations of subtle changes in resident's condition, i.e. behavior shifts, mood, skin coloring, appetite, etc. and takes action accordingly within the NA's assigned role.
  
• Participates in interdisciplinary team meetings, contributing to the identification of patient problems or needs.
  
• Verbally provides patient information to the treatment team for inclusion in the veteran/residents' plan of care, participates in treatment team meetings for resident care planning.
  
• Responds appropriately to emergency situations such as cardiopulmonary arrest; provides for environmental safety, security and veterans privacy.
