Job Description:
Administrative and Clinical Duties
Provides direct patient care to target population
Properly documents all patient interactions in the patients
electronic health record (EHR).
Electronically enters charges related to patient care in EHR.
Electronically reviews and signs off on laboratory results and
other patient related information in a timely manner
Electronically reviews and authorizes prescription refills (when
appropriate) in a timely manner
Electronically reviews and authorizes specialist follow up
referrals (when appropriate) in a timely manner
Electronically documents adherence to the health
maintenance/screening guidelines found in Care Resources Manual of
Clinical Care Guidelines, with particular attention to those
required for Universal Data Systems (UDS) and meaningful use
reporting
Electronically reviews and signs off on laboratory results and
other patient related information in a timely manner.
Maintains productivity standard set by management by ensuring
appointments slots are filled and accessible for emergency walk-in
appointments
Provides medical on-call coverage as required, if employed Full
Time. Will be on call at the same frequency as other providers, but
will share call with an internal medicine PCP who will cover adult
patients.
Complies with agencys policy as it relates to External Access to
Patients Electronic Health Records
Completes required documents/forms on behalf of patients
Promotes agencys ancillary services (i.e. Nutrition, Dental,
Psychosocial, Specialty Care) when clinically indicated and
appropriate.
Provides primary care to patients in their homes when necessary
Quality Assurance/Compliance
Assures quality measures are met HEDIS and UDS
Completes monthly Medical Quality Assurance assessments (QA records
review).
Completes all required trainings
Participates in regular case staffing to discuss procedures and
quality assurance with the medical team.
Safety
Ensures staff maintain agency guidelines relating to safety,
outreach and confidentiality
Ensures proper hand washing according to Centers for Disease
Control and Prevention guidelines.
Documents patients medications correctly.
Makes sure each patient knows which medicines to take when they are
at home and encourages each patient to bring their up-to-date list
of medicines every time they visit the doctor.
Ensures each new client receives screening for their risk for
suicide.
Understands and appropriately acts upon assigned role in Emergency
Code System.
Understands and performs assigned role in Agency Continuity of
Operations Plan (COOP).
Ensures out posted staff understand the safety requirements of the
location in which they are posted.
Other
Participates in agency developmental activities as requested.
Other duties as assigned.
