opportunity today! Responsibilities Hospice Aide Coordinator
Responsibilities: Coordinate the scheduling of Hospice Aide visits
with Manager, Clinical Team/Registered Nurse (RN) Supervisors to
meet and maintain the care plan requirements for patients/families
and the staffing needs of the agency. Make patient care assignments
within the parameters of the Hospice Plan of Care and visit strings
determined jointly with the Manager, Clinical Team and Director,
Clinical Operations. Work with the education department to develop
and implement Hospice Aide training both internally and externally.
Participate as part of the management team, Quality Assurance &
Performance Improvement (QAPI) Committee and other committees as
assigned. Coordinate and participates in Hospice Aide orientation
and competencies review. Responsible for: Management of ordering
and disbursement of medical supplies to the Hospice Aides.
Maintenance of the medical supplies in an organized, clean supply
room. Management of costs for required medical supplies to meet the
needs of the patients and within the budget requirements of the
agency. If necessary: Provide personal care, including bathing,
dressing, grooming, hair, nail, skin and foot care and oral
hygiene. Assist patients in turning, sitting, standing, and
ambulation, according to the plan of care. Assist with elimination,
including the facilitation of bedpans, and urinals, patient trips
to the bathroom, routine Foley care, and enemas, according to the
plan of care. Assist patients with meals, when necessary. Perform
simple procedures as an extension of therapeutic services, in
accordance with the interdisciplinary plan of care. Observe and
report changes in the patient's condition and needs to the Manager,
Clinical Team or Primary Registered Nurse (RN). May perform simple
homemaking chores for the patient directly, meal preparation,
and/or light housekeeping in the patient's immediate area. Complete
required documentation in a timely manner. Assist, observe and
mentor Hospice Aides in the field as necessary in coordination with
the RN supervisor. Acts as liaison between Hospice Aide, Manager,
Clinical Team and other clinical staff as well as facility staff
and patient/families Qualifications Hospice Aide Coordinator
Qualifications: Certified by the state(s) as a nurse's aide, or
CHHA, or CNGA. Board certified Hospice and Palliative Nursing
Assistant CHPNA preferred. At least two years experience in Hospice
care with strong communication and organizational skills. Must have
reliable automobile, valid driver's license, and the minimum state
required liability auto insurance.
