Job Description:
We are excited to newly offer the role of *Virtual Care Partner*
(VCP) for our inaugural Virtual First primary care program in Palm
Beach, FL. As a VCP, you will bring your *Medical Assistant
*skillset to become part of a virtual clinical care team in
partnership with a Virtual PCP to deliver a concierge healthcare
experience that engages our patients in lifelong well-being and
health. We're looking for tech-savvy individuals who bring the
heart and audacity to help us profoundly improve the healthcare and
aging experience for Medicare patients in the Palm Beach area. A
VCP is a deeply compassionate and curious relationship-builder,
creative problem solver, and empathetic listener with strong verbal
and written communication skills. The VCP will serve as the
bridge-builder between the patient and the Virtual PCP, especially
since the patient will not be able to meet the Virtual PCP
in-person, as they would for traditional brick-and-mortar primary
care.The Medical Assistant 2 performs clinical duties such as
discussing symptoms and gathering and inputting information into
the electronic medical records system, as well as care coordination
activities such as responding to patient calls and messages,
following up on patient care plans, and helping patients access
community support resources. Collaborates closely with Physicians
and Advanced Practice Providers. Delivers direct patient care
dependent on what active certification allows. *Job Functions**
This clinic is located at 1651 S. Congress Avenue, West Palm Beach,
FL 33406. This position requires the candidate to live within the
Palm Beach, FL area.* Answer questions from prospective patients
about the Virtual First program* Utilize software solutions (e.g.,
CRM tools) to manage patient registration into the Virtual First
program and chart creation within eCW* Build relationships with
patients and their families, and welcome them into the Virtual
First Program by facilitating a nimble onboarding process that
includes patient-centered goal solicitation, initial
biopsychosocial assessment, and rapid delivery on patient requests*
Act as the patient's (and family's) primary point of contact to
navigate their healthcare journey, responding to questions,
appropriately and expeditiously escalating clinical concerns, and
ensuring patients and families feel supported by and connected to
the care team at all times* Cultivate close relationships with
patients telephonically and virtually (via video, email, and chat)
to discuss progress, identify and address care barriers, and
facilitate ongoing engagement* As needed, join the home visit team
on their visits to local patients* Working closely with the Virtual
PCP to support behavior change, implement patient-centered
goal-setting techniques, and share digital educational resources*
Identify and connect patients to available clinical, behavioral,
and social resources both internal to Conviva and throughout the
patient's community* Complete clinical chart reviews, identify key
clinical information in past medical records, specialist and
diagnostic reports, and document patient information and vitals in
the electronic medical record (EMR)*Required Qualifications** An
active Certified Medical Assistant/CMA or Registered Medical
Assistant/RMA certification from one of the following
organizations: AAMA, AMT, ARMA, MedCa, NAHP, NAHT, NCCT, NHA or AAH
or willing to obtain certification within 1yr of appointment* 2 or
more years of Medical Assistant experience with direct patient
care* Experience in a fast-paced/high-volume environment* A unique
combination of skills including empathy, listening, customer
service, critical thinking, tech savviness, problem solving and
strong verbal and written communication skills* Strong technology
proficiency, including email, video conferencing technology, and
mobile apps* Experience with electronic medical records*Preferred
Qualifications** Bilingual in English and Spanish (proficiency in
Creole is an additional plus)* Experience with HEDIS* Ability to
utilize behavior change techniques, including motivational
interviewing, to help patients identify, understand, and follow
through on targeted goals* Experience working in a value-based care
setting*Additional Information**Schedule: Monday - Friday 8:00 AM
