Registered Nurse - Maternity Care Coordinator - West Palm Beach, Florida
Duties

Duties include but are not limited to:

• 
  
• Self-directed in assessing, planning, implementing, coordinating, monitoring, and evaluating patient care activities in relation to the cervical cancer screening/Paps; breast cancer screening mammogram and breast care services; maternity care process; and Women Veterans Program
  
• Demonstrates effective coordination of care of women Veterans requiring maternity care and interval assessment of care in accordance VHA Handbook 1330.03
  
• Demonstrates effective coordination of care of women Veterans requiring cervical cancer screening/Paps/HPV tests, test tracking and care coordination with Women's Health Providers
  
• Demonstrates effective knowledge of care of women Veterans requiring mammograms, breast ultrasounds, breast MRI's, breast biopsies, surgeries or other breast care
  
• Demonstrates effective coordination of care of women Veterans requiring Specialty Care and/or testing at other VAs within the network or through CITC consults
  
• Maintains database of all women Veterans with scheduled, cancelled, no show appointments, with follow up pertaining to mammogram program; Pap's, HPV tests, and utilizes the information to make changes in the services
  
• Maintains database of all pregnant women Veterans to monitor the delivery of maternity services and coordination of care and tracks maternal and fetal outcomes
  
• Serves as a resource to ancillary and support services to control and prevent inappropriate mammogram consults, thus decreasing duplication of services and no-shows, increasing utilization of appointments, and increased patient satisfaction
  
• Applies the nursing process or processes at the unit/team/work group level to improve patient care
  
• Provides a full range of nursing care to patients with complex care needs utilizing the nursing process
  
• Completes initial assessments and re-assessments of patient condition and response to treatment and other clinical findings; Initiates follow-up as needed
  
• Accurately documents in the medical record patient findings, assessments, and care provided
  
• Applies problem-solving skills, on the unit or Program level, leading to an improvement in patient care
  
• Demonstrates leadership by involving others in improving care
  
• Acts as a patient advocate when patient self-determination is in question
