Home Health is a licensed Florida home health agency dedicated to
providing compassionate, high-quality care to patients in the
comfort of their homes. We support independence, dignity, and
safety while ensuring compliance with all state and federal
regulations. Licensed Practical Nurse Home Health Employment Type:
Full-time / Part-time Schedule: Weekdays & Weekends required
Responsibilities: Follow comprehensive patient assessment E1 and be
able to review and interpret individualized care plans. Provide
skilled nursing services (medication management, wound care, IV
therapy). Educate patients and caregivers on disease management,
medications, and safety. Document visits accurately and in
compliance with state and agency policies. Communicate with
physicians and interdisciplinary team to update care plans.
Participate in weekend/holiday coverage. Requirements: Active
Florida LPN license. Current CPR certification, TB test, valid
drivers license. At least 1 year of clinical experience (inpatient
and home health required). Valid drivers license and reliable
transportation. Must carry Liability insurance Clear background
check and eligibility to work in the U.S. Ability to work flexible
hours including weekdays, weekends, and some holidays. Strong
communication, organizational, and problem-solving skills.
Preferred Qualifications Previous experience in home health or
long-term care. Woundcare and Woundvac experience is a plus Iv
antibiotics administration experience is required Must be able to
perform one on one with patient Must be able to do blood draws
Familiarity with EMR systems. Bilingual (English/Spanish or
