Description:
The Intake Clinician is responsible for performing a wide variety
of intake assessments as directed by the Clinical Director. He/She
will collaborate with the Treatment Team to formulate the Initial
Treatment Plan and assist in coordinating continued care plans. The
Intake Clinician will also fill in as needed as a Primary
Therapist.
SPECIFIC AREAS OF RESPONSIBILITY TO POSITION
Review assessments from referring facilities and note
problems/issues that require further attention.
Complete the following intake assessments and documents within
required timeframe set by CD:
Bio-psychosocial Assessment
Bio-psychosocial Update (when needed)
Suicide Risk Assessment
Spirituality Assessment
Trauma Assessment
Educational Learning Assessment
Initial Treatment Plan
Admission Progress Note
Introduce client to facility and staff members in a professional
manner.
Gather comprehensive diagnostic information through psychosocial
assessment interview then develop an Integrated Summary, bringing
together information from all assessments to identify problem areas
that may affect treatment and/or need to be addressed in treatment
planning.
During first session interaction with client, forward any pressing
matters to Clinical Director for further evaluation if needed.
Maintain clinical records according to program policies and those
of licensing and accrediting agencies, documents accurately as it
relates to treatment goals and objectives, ensure that
documentation is timely, consistent, clear and articulate and keep
clinical records secure at all times.
Lead and co-facilitate group therapy sessions in a manner that
creates an atmosphere in which self- disclosure is facilitated,
helps patients gain group process skills, and helps patients gain
insight into problems and utilizes experiential techniques
effectively when instructed by CD.
Assist clients in answering any questions regarding program and
what treatment will look like.
Adapt to different types of clients.
Maintain healthy boundaries.
Understand and apply the program's clinical philosophy.
Demonstrate good strategic and problem-solving skills.
Accept and utilize supervision and address problems noted by
Supervisor.
Communicate with Supervisor and keep Supervisor informed of
significant client issues.
Have good work habits: get things done in a timely, efficient
manner, adhere to time schedules, maintain good communication,
demonstrate willingness to accept responsibility and function well
as a team member and contribute positively to the morale of
staff.
Responds and assists in medical emergency situations as directed
and needed.
Adheres to facility policies and procedures, DCF Rules and
Regulations and JC Standards.
Performs other duties as assigned.
This is a 1099 Contactor position.
Pay Range $30.00 to $50.00 Hourly commensurate with experience and
licensure
.
Requirements:
EDUCATION:
Master's Degree in social work, psychology or related human
services field, or Certified Addiction Professional License.
EXPERIENCE:
Two years of work experience in the behavioral health care
setting.
LICENSURE/CERTIFICATION:
Licensure and/or (Intern License) to practice clinical discipline
in the appropriate state, as applicable, required in Social Work,
Marriage and Family Therapy, Mental Health counselor and/or
SKILLS/ABILITIES:
Demonstrates Proficiency in Communication, observation & Written
skills.
Knowledge of State & Federal Statutes Regarding Patient
Confidentiality laws.
Knowledge of Drug-Free Workplace Policies.
Knowledge of Workplace Violence.
Knowledge of Corporate Integrity & Ethics
Knowledge of addictions.
Ability to interact with a multi-disciplinary team to assist in
facets of the recovery program relating to physical health.
Knowledge of the 12-Step Recovery Program.
Knowledge and competency in problem solving, stress management,
ethics and team building.
Knowledge of alcohol and other drugs that includes;
- Alcohol and addictive drugs and their physical, emotional,
intellectual, and spiritual impact on the individual.
- Alcohol and addictive drugs and their impact on the family.
- Ability to determine if a medical emergency exists and to take
appropriate action, when necessary.
- Knowledge of emergency procedures used in case of alcohol and/or
drug overdose.
- Knowledge of First Aid and CPR.
- Knowledge of Federal, State and Municipal laws and/or regulations
that regulate the treatment of alcoholism and drug addiction,
including:
- Confidentiality
- Patient Rights
- Professional Code of Ethics
- Discrimination
- Flexibility to adapt to schedule changes and assumption of
responsibilities not delineated in the job description which are
related to work as a member of an addiction treatment team..
