. The Department of OBGYN has an exciting opportunity for a
full-time Patient Navigator to work in Miami, FL. The Patient
Navigator facilitates patients' ability to obtain services by
identifying barriers to care, coordinating the development of
treatment plans, and monitoring the implementation and follow-up of
services ordered by caregivers.+ Conducts patient evaluations to
obtain information necessary for the development of an effective
treatment plan.+ Ensures patients have access to insurance and
social service programs for which they are eligible.+ Provides
patient reminders of upcoming appointments and acts as liaison
between patients and providers.+ Identifies and resolves real or
perceived barriers to care.+ Educates and counsels patients on
issues related to their health and life situations.+ Maintains
current knowledge of community health, welfare resources, and any
other programs available to patients.+ Adheres to University and
unit-level policies and procedures and safeguards University

diploma or equivalent+ Minimum 3 years of relevant work experience+
Ability to process and handle confidential information with
discretion.+ Ability to communicate effectively in both oral and
written form.+ Ability to handle difficult and stressful situations
with professional composure.+ Ability to maintain effective
interpersonal relationships.+ Ability to understand and follow
instructions.+ Patient navigation is at the core of the success of
this program, providing streamlined, patient-centered access to
medical care, and, to mental health services.+ The Patient
Navigators will promote health equity and access to systems of care
for survivors of human trafficking. This person will ideally be a
peer with similar life experiences as our survivors to enhance
survivors' safety and long-term recovery efforts.+ Primary
functions include helping survivors successfully access medical,
mental health and psycho-social support services; providing the
navigational support to do so; serving as the liaison between
survivors and the clinical teams to improve adherence to medical
care plans, health literacy and outcomes; and, connecting survivors
with appropriate social services including housing, employment,
insurance, education, transportation, and others.+ The Patient
Navigator will coordinate clients care with the clinical team and
accompany them to every medical appointment.+ Patient navigators
will be trained as doulas and will be able to support prenatal,
delivery, and postpartum care for pregnant clients.+ Patient
Navigators are the keystones to the success of THRIVE.Any
